Items required for new student registration: Frequently Asked Questions

 

PLEASE READ EACH STEP 1-3 COMPLETE ALL FORMS. MAKE AN APPOINTMENT TO COMPLETE REGISTRATION AT THE DISTRICT OFFICE AT THE END OF STEP 2 PRE-ENROLLMENT CONFIRMATION PAGE. THESE LINKS ARE ONLY FOR THE CURRENT SCHOOL YEAR THAT ENDS IN JUNE 2017.

 

 

PLEASE PROVIDE ORIGINAL DOCUMENTS PLUS ONE PHOTOCOPY OF THE FOLLOWING TO EXPEDITE REGISTRATION:

 

(1) Proof of Residency (3 groups). Note: If you are a new resident, you will have thirty (30) days to provide group #3.

 

(2) Parent/Guardian Indentification

 

(3) Student Birth Certificate or passport

 

(4) Immunization records: Immunization Requirements

 

  • STEP 2: On-line Pre-enrollment link. Please print out confirmation, and make an appointment online for in-person registration at the District Office 3477 School Street. You may also email msibley@lafsd.org or call 925-927-3584.

Intra-district transfer requests will be accepted at the district office 2/1/18 - 3/30/18
