Lafayette School District offers a generous benefit program.  This Summary provides information about the insurance plans offered to eligible employees. This includes plan summaries highlighting the most common medical services, plan eligibility, dependent coverage, and premium rates and your contributions.  Premiums will vary based on whether you are full-time or part-time and according to the plan and number of dependents you enroll.

 

Kaiser HMO - The District will pay 100% of the Kaiser Single, Two Party and Family premiums for full time employees, except for psych interns, confidential employees and administrators.  KP Plan Summary

 

Anthem HMO - The District will use an annually determined Kaiser composite rate to calculate its share toward an employee’s monthly premium. The District will contribute 100% of the composite amount (District cap) per month for a full time employee. The full-time employee will pay for the difference when the selected plan’s premium exceeds the composite rate. For part-time employees, the District’s share of premiums will be a prorated amount of the composite rate. ABC Plan Summary

 

Delta Dental - The District will contribute 100% of the composite rate per month for a full-time employee. For part-time employees, the District’s share of premiums will be a prorated amount of the composite rate.

 

Vision (VSP) - The District will contribute 100% of the employee only rate per month for a full-time employee. For part-time employees, the District’s share of premiums will be a prorated. An employee may obtain this vision coverage for eligible dependents at the employee’s expense.
